Clinical Services Unit Social Worker/Case Manager

The Taylor County Human Services Department has a full-time opening for a Clinical Services Unit Social Worker/Case Manager. The Clinical Services Unit Social Worker/Case Manager will provide case management services, outreach, assessments, develop case plans, initial screening, coordinate diagnosis/assessment, rehabilitation, treatment and support services, provide crisis intervention services and maintain a working knowledge of the legal rights of people affected by severe and persistent mental illness and/or substance use disorders and maintain client case records as required.

Applicant must possess a Bachelor’s Degree in Social Work, Human Services, Psychology, Nursing or closely related field. Certification or eligibility for certification as a social worker or registered nurse in Wisconsin, as well as, two years of work experience in service delivery to people with severe and persistent mental illness and/or substance use disorders is preferred. A valid driver’s license is required. This position may be allowed remote work for a portion of the work schedule, depending on the needs of the position, organizational and work unit requirements and employee’s preference.
